Issue Severity Definition
Issue severity is calculated based on a combination of Risk and Probability. Elements of risk that are evaluated include Environment, Injury and Regulatory Risk. Each issue is scored by a team of EHS professionals using the below calculation standard.
- Environment Risk
- 0 – No risk
- 1 – Minor Property / Environment Damage
- 2 – Significant Property / Environment Damage
- 3 – Major Property / Environment Damage
- Injury Risk
- 0 – No risk
- 1 – First aid or minor medical treatment
- 2 – Significant Injury
- 3 – Death or permanent disability
- Regulatory Risk
- 0 – No risk
- 1 – Low dollar citation
- 2 – Medium dollar citation
- 3 – High dollar citation
- Probability
- 1 – Low
- 2 – Medium
- 3 – High
An overall severity is technically calculated based on the above. We take the maximum value of all the risk factors and the sum is added to the Probability factor. This total sum is divided by 2.
- If the result is >1, Severity = 1 (Low)
- If the result is 1>2, Severity = 2 (Medium)
- If the result is >3, Severity = 3 (High)
